१. सूत्रस्थानम् 1.sūtrasthānam,-१आयुष्कामीय:-01āyuṣ-kāmīya:, (S.-1, Ch.-1, V.-34)

त्यजेद् आर्तं भिषग्-भूपैर् द्विष्टं तेषां द्विषं द्विषम् । हीनोपकरणं व्यग्रम् अ-विधेयं गतायुषम् ॥ ३४ ॥ चण्डं शोकातुरं भीरुं कृत-घ्नं वैद्य-मानिनम् ।

tyajed ārtaṃ bhiṣag-bhūpair dviṣṭaṃ teṣāṃ dviṣaṃ dviṣam । hīnopakaraṇaṃ vyagram a-vidheyaṃ gatāyuṣam ॥ 34 ॥ caṇḍaṃ śokāturaṃ bhīruṃ kṛta-ghnaṃ vaidya-māninam ।

त्यजेद् tjadžéd = reject; आर्तं árta = for; भिषग् bhiṣag = healer, vaidhyaभूपैर् bhúpair = king; द्विष्टं dviṣṭaṁ = unfriendly, hateful; तेषां téṣáṁ =this, that; द्विषं dviṣaṁ = to dislike, to hate; द्विषम् dviṣam = to be hated;

हीन hín = insufficient, weak; उपकरणं upakaraṇa = required instrument; व्यग्रम्∼vjagra = distracted, scattered; अ-विधेयं∼a-vidhéja = inappropriate treatment; गता gatá = approach, to approach; आयुषम्∼ájuṣa = for life;

चण्डं∼čaṇḍa = angry, ardent, cruel, cold-blooded, malicious; शोक śóka = sadness, grief, sorrow; आतुरं∼átura = desirous, ill; भीरुं∼bhíru = fear; कृत-घ्नं∼kṛta-ghna = defiant, ungrateful; वैद्य vaidyja = the Ayurvedic practitioner; मानिनम् máninam = opinion

Patients unfit for treatment

Tyajya rogi are patients whose treatment is rejected, i.e., patients condemned by the king, rebels against the king, those who are against the Ayurvedic practitioner, who lack sufficient resources for the treatment, who do not take the treatment seriously, who do not believe the Ayurvedic practitioner's advice, who are angry, who grieve for the loss of a close person, who are afraid of Panchakarma, who are ungrateful and who consider themselves to be the physician.



Commentary

This verse was written a long time ago, when Ayurveda was controlled by the king, and from this point of view such behaviour was natural. Nowadays, the ethics of the physician is a little different.

When people show off, they are displaying their likes and dislikes, everything about their ego. This is different from being a responsible person who can experience repentance. Everything that we do impacts others and we should think our actions through before we do them.

There is a difference between Ayurveda, modern medicine, and social welfare systems. In Ayurveda, the physician has the right to refuse to treat somebody. In a social welfare system, everybody has to be kept alive until the last possible moment, and in modern medicine, if the money is available, then that is what will be done. We know that among some monkeys, when a member of the group dies, they sometimes drag the corpse around with them after death, and that is a bit like what the modern system allows, the perpetuation of that kind of attachment.  

Patients who reject the Ayurvedic practitioner or who hate the ruler of the territory are not appropriate for Ayurvedic treatment. You should not bite the hand that feeds you. In Hindi the saying is to drill a hole in the plate in which you serve somebody food. It means the person is not faithful to those who are taking care of him or giving him an opportunity to make money and to live. If a practitioner discovers that the patient is like this, then he is not obliged to treat him. 

Also people who are unable to focus, when they are distracted and unable to concentrate, the practitioner will not attempt treatment. Such people also will say they can't afford to do what needs to be done to cure the illness, or they will not be able to decide whether to do it or not, or they decide that they are fine as they are and don't want to cure the illness and ask to be left alone - so that is what happens.

Also people who are either very malicious and negative or people who are overwhelmed by grief are not appropriate candidates for treatment. People who are addicted to their own fear, or who fear the procedures prescribed, and people who are defiant and ungrateful are also not to be treated.
