Difficult to cure diseases |
Krchha sadhya (difficult to cure) diseases are those that require Shalay (surgery), Kshar (special Ayurvedic preparations), Agni karma (cauterization), the use of potentially toxic substances, etc., and diseases where there are none of the factors mentioned for a Sukha sadhya roga (as in the previous verse). |
Commentary The sutra clearly implies how important it is that the Ayurvedic practitioner estimates the scope of the diseases and determines a treatment on that basis. Some diseases may attack the tissues so that they become so dysfunctional that it is impossible to recover. In such a case it is important not to proceed recklessly but very accurately, step by step. We may call these diseases complicated if they involve chronic inflammations, tumors etc. It is necessary to use extraordinary treatment methods for them. Today in Europe, to use these Ayurvedic treatment methods is out of the question, Kshar and potentially toxic substances are not allowed for use in treatment. Therefore the Ayurvedic practitioner shold not admit patients with Krchha sadhya diseases or try to treat them, as it is illegal. These patients have to be treated by a medical practitioner, using the surgery of the modern medical science that is approved. It is sad that the Ayurvedic practitioner may not fully provide what Ayurveda has to offer and that the European citizens may not use Ayurveda in its fullness. If the symptoms of the disease are of multiple kinds and are confusing, and if the use of surgical tools is required, such diseases are considered difficult to cure. A disease that is easy to cure will resolve quickly. One that is difficult to cure will take a long time to resolve. We recall the prakrtis and their symptoms. When the practitioner discovers that the symptoms are all related to Vata, then that guides the treatment. However, if more than one dosha is involved, that will be a more difficult disease to treat, because when we influence Vata that will influence Pitta and we must gradually, slowly attempt to control the changes so we don't cause the disease to spread further. There will be counter-indications as to what not to use. When the situation changes, the treatment must be adapted accordingly. We focus on what is the most acute problem at the time. When the patient is disciplined and dedicated to recovery, then the disease is curable. People are able to overcome even the loss of limbs and carry on if they have the will to live. If the patient loses faith in the doctor, that becomes a difficult to cure situation. If there are not enough resources available - money, materials, mental resources, all of the conditions necessary to treatment - then the disease will be difficult to cure. If the medication required is unavailable or too expensive, that means the treatment cannot happen. |
